Ectopic Pregnancy-Exceptions to the hCG Level Rules & New Recommendations

Video taken from the channel: Steven Palter

In the event of a miscarriage, hCG levels typically decrease from previous measurements. For example, a baseline level of 120 mIU/mL that’s dropped to 80 mIU/mL two days later can indicate the. With serial hCG measurements, quantitative hCG blood tests are drawn two to three days apart. This is because ordinarily, in early pregnancy, the hCG level in your blood doubles every two to three days. If your hCG doubling time is slower than expected, or if it decreases over time, this may be a sign of a miscarriage or an ectopic pregnancy.

Women whose hCG level falls over a period of two to three days in the first trimester in two quantitative hCG blood tests are often advised that this means an impending miscarriage. This is especially true for women with other miscarriage symptoms, such as vaginal bleeding during pregnancy. 4 . hCG Levels After Miscarriage After suffering from a miscarriage, your hCG levels will continue to decrease.

They should return to normal in about 4-6 weeks, depending on how far along the pregnancy. Miscarriage is a pregnancy loss before 20 weeks of gestation. Low hCG levels can sometimes indicate that you have had or will have a miscarriage.

Normal levels of hCG vary between women and the stage of pregnancy however so low levels of hCG are not always a sign of miscarriage. Does slow rising hCG mean miscarriage?hCG hormone is likely to suppress your normal ovulation process. Your next cycle may start only when the hCG levels drop below 5mIU/mL (6).

It is likely to be normal for the pregnancy test to show positive following a miscarriage. It takes time for you to cope with the loss, but you will soon get back to normal. hCG levels at the moment of miscarriage may vary depending on how many weeks pregnant the woman was when she miscarried.

For instance, hCG levels at week six—i.e. two weeks after the first missed period—range from 1.080 mUI/ml to 56.500 mUI/ml. Human chorionic gonadotropin (hCG) is a hormone that the placenta makes during pregnancy. It tends to rise through the early stages of pregnancy, doubling every two to three days during the first four weeks or so of gestation and peaking during weeks eight to 11 of gestation. If you miscarry, your level of hCG will gradually fall. A low hCG level can mean any number of things and should be rechecked within 48-72 hours to see how the level is changing.

A low level can indicate: Miscalculation of pregnancy dating Possible miscarriage or blighted ovum. My hCG level (the hormone that indicates a pregnancy) was on the low end for someone 4 to 5 weeks along and the ultrasound showed no evidence of a baby. It was possible I was having a miscarriage, and I would need more tests to confirm it.

But here’s the thing about pregnancy: every symptom can be interpreted in different ways.

List of related literature:

Levels of hCG fail to rise appropriately in ectopic pregnancy, whereas abnormally high levels of hCG in combination with vaginal bleeding can indicate a molar pregnancy (although occasionally a multiple gestation may also account for the high levels).

  • What I don’t understand is, at 5 and 1/2 weeks, why can’t a gestational sack be seen in a viable pregnancy? Why does the sack show up much later in some women? Can’t there be a better ultrasound technology that can check for viable/tubal pregnancies? I have been reading a lot of comments on websites and some women say they were misdiagnosed because no sack could be seen around 6 weeks. Where is it? Why does it take longer to show up?

  • So sad that it happened… My prays for you!
Try again, don’t give up! You have wonderful son! I was 5 month when I lost my pregnancy and love for my daughter helped me live through and accept it.

  • Hi. Im in very early pregnancy hcg of 11dpo 16, 13dpo 49 and Progesterone of 38. The dr at the hospital had a quick bedside scan and said it maybe eptopic. However reading research. It states progesterone over 25 is 97% a viable pregnancy not eptopic. Is this true? Ive been left to wait another anxious 4days before having another scan.

  • So Mr Palter I was 9 weeks and they could see my baby in the fallopian tube My hcg levels were 41,000. They said they could see no heartbeat but only did one ultrasound. Anyway i could have been misdiagnosed?

  • Watching this made me emotional. My husband and I have been trying to conceive and finally saw a specialist. We had a IUI done on May 21st and yesterday I found out through blood test that I was pregnant! I definitely was not expecting to hear that. The nurse at the fertility dr. Basically told me to prepare myself for a miscarriage since my levels are so low. But I know God is powerful and will make a way even if we don’t see one. I love your channel and found hope in watching this video.

  • So sorry to hear that! Hope everything goes well with healing and soon enough you and your husband will have a nother bundle of joy.

  • Oh no, honey, I’m so sorry. We’re all here for you. You are being so brave to share this with us it really makes us feel not alone. I know at times this may be incredibly hard to do, but I wanted you to know it’s appreciated and we love you through it all.

  • I just got my period 6 weeks post miscarriage. We were not preventing during the weeks after I miscarried, but we didn’t get pregnant surprisingly. Thankfully getting my period means I can try again rather than being a huge disappointment this time! I am trying to stay positive and not feel like I can’t be excited or get attached this time!

  • 12 weeks…I was immediately set up for surgery and my husband and mother barely got there went they rushed me into surgery…they removed my tube, my ovary and my uterus…I want to know if they could have harvested my eggs and without the tube could they have still fertilized those eggs and planted them into my uterus…I feel so robbed of being able to have kids…I know the technology wasn’t all there yet..but surely there could have been another way?

  • It may take 2 spaces to explain…I had an ectopic in 1980 it was in my right tube…I already had my left ovary and tube removed because of an ovarian cyst…at the time of my ectopic…I had no idea I was pregnant and went to work with pain…I brushed off until it got so severe I had to leave and go to the ER…when I did the first question they asked was ARE YOU PREGNANT?.I said I didn’t think so…they gave me a pregnancy test and an ultrasound…and sure enough I was and I was approximate

  • I know this is 6days ago im so sorry sweetie ive watched a few of your youtube videos and how much you wanted a sibling for your little boy

  • pain in the setting of IUD and pregnancy can be miscarriage but ectopic pregnancy must always be considered even if the levels are going down. Please seek evaluation by a gynecologist or if unavailable return to the ER

  • My hcg was 370.9 at 6 weeks nothing in ultrasound. 72 hours later it is 715.9 still rising. I still feel cramping and nauseous. No bleeding I am on lovenox. Can this still be viable

  • I came to know that I am pregnant on 22 Sept this year. We were so happy to have our second child. after two days I experienced severe pain in abdomen. At first I thought it could be gastric problem and I should take some rest. When after half an hour situation did not change, we rushed to hospital. After examining, doctor asked us to do beta-HCG serum test and sonography. Reports indicated ectopic pregnancy. since HCG level was 2300 and I was stable, doctor suggested for medical treatment, also explained deadly side effects of this injection. They gave me Methotrexate.  After 4 and 6 days they repeated Beta HCG test, it was showing pregnancy is still on. Methotrexate affected neither me nor pregnancy. And finally I had to go through laproscopic removal of right fallopian tube. Now I am on the way to recovery and too scared to have pregnancy again.

  • Hi am 8 weeks pregnant or was I am not sure went to the ER on 11th and my hormone level was 330 went back 3 days later and it was 338 went back 3 days later and it was 174 they never did a pelvic exam or a vaginal ultrasound told me I was having a miscarriage because I was having bleeding they never checked to see if it was an ectopic pregnancy or not and I have no idea if I am having a miscarriage or not or if I have an ectopic pregnancy no one will give me answers. What is your input on this?

  • Thank you for this wonderful detail information. I was just wondering if it is also possible to diagnose it using CAT scan or MRI checking to see if there is abnormal growth of tissue around or inside the fallopian tube? Just curious.

  • I got pregnant 2 months after my d&c. I unfortunately lost that baby too at 11 weeks. Baby stopped growing at 8 1/2 weeks. So yes getting pregnant after a d&c def something that happens often that I have read and heard. Love and light girl! Oh and I still bawl when I drive alone or in the night when every one is sleeping (my only alone time) I just heard the song by lady gaga called “til it happens to you” and I know it was originally written about sexual assault, but when I hear it, it’s about how i feel about my losses. Its intense but the words can totally heal. I’m not even a huge lady gaga fan but those words can tell a story. <3 <3 <3 sorry for blabbing on so long:)

  • I’m so sorry sweet girl! Praying for you and your fam. I know it doesn’t make it any easier but I have had 3. It’s the toughest thing but the Lord will see you though. I know you will get your rainbow baby very soon. You are so strong ❤️

  • Consider yourself lucky gf! Where I live, prenatal care really isn’t priority and they make you wait WEEKS to get any bloodwork or anything! Sometimes 8-12 weeks. It’s super frustrating. But so happy to be here for this journey♥️♥️♥️♥️

  • I completely understand you! With my first pregnancy it just happened for us, and I didn’t have to worry about HCG levels. Last month I had a chemical pregnancy while still seeing my Fertility Specialist and I have never stressed so much within that week just wondering if I am pregnant or not, and sadly lost my pregnancy:(. However, I’m so happy for you!

  • best research ever for a pregnant woman to know especially those with delivery difficulty in pregnancy or ivf cases, my hcg levels are now 1996 and in week7 but dr not encouraging he says its up normal and will pass later and stop fertility medication, im still hopeing for best, 1st pregnancy after 6 trials of ivf, no strong cramps or bleeding, im praying god it will continue

  • I’m sorry for your lost. I know
this hurt too well. I lost my son at 6 months pregnant. He was born sleeping at 23 weeks and we got pregnant again 2 months after. Had a missed miscarriage at 5 weeks. It’s been 5 months since our miscarriage. I want to be pregnant so bad. Watching this was hard. Today is exactly 1 year of finding out I was pregnant with our son that was born sleeping.

  • I’m 12 weeks and bleeding heavy cramps I went to emergency did blood test and ultrasound. Dr told me hcg are low I have to go back tomorrow for another blood test to see if it raising. I’m praying God to give me this child as a testimony may His will be done.

  • So you’re saying an ED physician shouldn’t definitively tell you you’re having a miscarriage when they don’t really know, right? Because that is what happened to me at 2 different hospitals and it turned out I had an ectopic pregnancy. By the way, it is EXCRUCIATINGLY painful!

  • im so sorry Heather <3 i know exactly what youre going through. I just miscarried 2 days after you posted this video at around 8-9 weeks. Let yourself grieve the loss as i am doing the same. God will get us through it! Prayers that we both will have our rainbow babies soon xoxo

“I wish I knew what to say but I just don’t know what I could possibly say to make you feel better. I am here for you if you need to talk or anything #morgansfamily

  • Oh hun:( my heart is breaking for you. I am praying for you. I have had 8 miscarriages so i know what you are going through. Huhs

  • I’m so sorry you guys are going through this. My husband and I just experienced a miscarriage back in November. I am praying over you and your family. It is the most devastating thing to lose a child, even at five weeks which is when I had mine. We fall in love with our babies as soon as we get those positive pregnancy tests as parents/mom’s. I am so sorry you guys are also experiencing this. It was the most awful thing we had gone through. Take this time to grieve and get through this with your husband/family.

  • I’m in the same boat right now. Waiting on bloodwork. Tests are still positive. Believing and receiving in Jesus name. I also hate being around negative doctors. I have God on my side.

  • I went through my first Mc last month trying for baby #2 we also tried for 6 months… I was 8 weeks but baby stopped growing at 5 weeks it was the most traumatic thing of my entire life.. I’m so sorry for your loss…

  • I am currently 19 years old. I am not trying to conceive nor trying to prevent a pregnancy our motto is what ever happens, happens.
I have had two miscarriages and two chemical pregnancies. It’s absolutely heartbreaking I feel you girl.

  • I am so sorry for your loss Morgan Family! Prayers for you !! I’m so sad as well !! I’ll be praying for you and when you least expect it you’ll will have a rainbow baby soon! Love y’all ♥️!

  • Oh my dear Heather. I had an early miscarriage in May of 2018 in much the same way, faint lines and then fading lines, then a heavier period once hcg reduced enough. Its extremely common for this to happen and it doesn’t mean there’s anything wrong with you. Reproduction is really luck of the draw and I’m sure you’ll get your lucky rainbow baby soon. It took another 6 months to conceive with our current pregnancy, I’m now 14 weeks. Hugs.

  • I am so sorry. I am sooo sooooo sorry. And I know you probably don’t want to hear this, but they do say that after a miscarriage, you’re more fertile. Please don’t let this discourage you. My heart is with you and it really must suck to have to go through this. ❤️❤️❤️

  • I am so sorry for you and your husband’s loss, Heather. It is okay to cry, greave, be angry, or even ask the good Lord above why, but know this that he is always there with us through the good and the bad. I remember when I lost my son at 20 weeks, and I went through a rage of emotions. I wasn’t ready to try until 6 months later, and I gave birth to a healthy boy 18 years ago. Take your time, and when you are ready to try again, then you will know. You are in my prayers.

  • When I had a miscarriage back in June at 9wks it took me exactly 1 month of weekly blood tests to get to 0. I though had a medicated miscarriage instead of a D&C, I’m prone to scarring, so hopefully it doesn’t take that long for you as well. I also have PCOS and it took me three months to get my period after

  • I am praying for you and your family to get though this hard time. God please be with this family and help them heal from this tragic time. Wrap your loving arms around them and let them feel your love that you have for them to let them know that you are there watching over them. God bless this family and everyone that watches this video that is going though the same thing or has gone though the same thing. It is in your wonderful name we pray Amen!

  • Thank you so much for this video. I feel I may have a misdiagnosed miscarriage, which my Dr. now thinks is ectopic, and this video has helped prepare to be a better advocate for myself and my pregnancy.

  • Yay! I’m sorry you had that thought/feeling with your fertility clinic. Mine was the opposite the first visit there was a girl newly pregnant and so happy and it gave me hope. I had an ectopic last month with my RE and they were so positive in the beginning with my numbers doubling and very supportive when things went south. My regular OB already told me they wouldn’t do 2 betas or even check my progesterone early on! Which really bummed me out. If we try again I will definitely go through my RE again. I’m glad your OB was very supportive for you!!

  • Did ur levels go up (progesterone)? I too have pcos & low progesterone. Im 6 weeks pregnant and light brown spotting so I’m extremely nervous

  • I did HCG 2 weeks it was negative the level are 5 a 50 to 10,000 a 100.000 i have symptoms i am worried i dont understand can pregnancy occur after going for HCG test when it show nagative

  • I had a chemical a year and a half ago. I got a positive a couple days before my period, the lines got a little darker, then they gradually got fainter and I got my period. It sucks and it’s not fair, but we gotta leave it in God’s hands. I’m sorry:/

  • Dr. Palter
Thank you for this excellent video. Recognizing the variability among individuals is especially useful for patients in my practice dedicated to tubal ligation reversal where each woman is considered to be at high risk for ectopic pregnancy until proven otherwise.

  • My heart breaks for you! Sorry for your loss I’ve had 8 loss and no births it never gets easy. I pray that your next pregnancy is healthy 😉

  • I am so sorry about your miscarriage. The Lord has His hand on your through this. Praying for y’all and that you can find comfort during this tough time.

  • Iam sorry about your misscarige i myself been threw two misscariges being trying for six months to concive and nothing yet. But at least we got our little one that make our day

  • I’m so sorry you’re going through this. I know how hard it is and how much it hurts. I had an early miscarriage in July, very similar to what you’re going through. But now I’m 12 weeks pregnant and everything is going great. Don’t lose hope, you will get your rainbow baby! You are in my prayers, sending you so much love during this tough time.

  • I had an ultrasound yesterday sep 3, 2020 and they saw the sac in the uterus but to early to see an embryo or heartbeat. My hcg levels were 1297.69 Monday August 31st, and only went up to 2199 by Thursday sep 3, The Nurse practitioner called me because they also saw what they think in could be a cyst or an ectopic pregnancy in my tube even though they saw the sac in the uterus. She didn’t feel like my levels are raising high enough. I don’t have any cramping or bleeding but I don’t have a lot of nausea or strong pregnancy symptoms. Any advice? I’m worry because I had a stillbirth this past January2020. Any words help even if is bad I need to prepare myself. Thank you

  • Hello! I wanted to let you know I found your channel about a month ago and I wanted to say thank you for sharing your journey. I suffered a miscarriage at 5 weeks three weeks ago. My HCG went away pretty quickly. I’m sure it’s because I wasn’t as far along. As soon as the bleeding stopped I started to track my ovulation and LH levels again. This week I got my peak! Hopefully these next few weeks are positive for both of us! thank you for sharing. It helps a girl like myself not feel so alone after going through something like this. ♥️

  • I’m so sorry about your news. I really am. But just continue to hold on tight, keep god in your prayers…..and pray to him everynight that he’ll bless u with another little miracle

  • This video gives me hope! I’ve had a ectopic pregnancy and lost my tube. Then I had a successful pregnancy. Now I’m pregnant again 4 weeks my hgc levels reach 500 but have decreased to 348 in a week no bleeding no pain but I’m hoping for a miracle my levels will increase again and not be ectopic!

  • Ohhhhhh Myyyyy….I am so sorry sweetheart. My heart breaks for you. Don’t give up baby. Stay strong. God doesn’t make mistakes and he has a reason for everything that he does. • To everyone reading this please turn to Jesus He loves you and wants to save you from what you are going through, Only Jesus can truly heal you, He is the God of miracles, The bible tells us in Romans 10:13 For whosoever shall call upon the name of the Lord shall be saved, I seen God do amazing things in my life and in the lifes of people I know, God saved someone I love from illness leading to death, There is nothing impossible to God, Its as simple as asking Him to show you He is there and if you really want to know Jesus will answer you, I know this because it happened to me when I called out to God
